Church Delegate fees will be assessed based on the total number of delegates registered here. (Church members, guests, or ministers not listed on this form will be required to complete a separate individual registration form and pay their fees personally). If a church is not going to pay for any of its delegates, simply place "0" in the appropriate box. This list confirms delegate eligibiltiy and will be the verification document used at check in when delegates arrive.

Church Checks are authorized for acceptance by mail or at checkin the day of conference. Mail in checks must be received no later than May 15th. If a check has not been received by that time and is not recorded at the time of arrival a credit card payment or another check must be presented to receive delegate badges and meal wristbands. Make all checks payable to the CMCHQ and annotate with General Conference Delegate Fees. Mail to PO Box 9 Florence, MS 39073.
