This form is for the individual delegate. Delegate names must be listed on the GC Church Delegate Form provided by each Church to receive a Delegate's Pass. Payment is due upon registration unless the Church has prepaid or committed to pay the registration on the delegates behalf.

I understand that registration and meal fees are non-refundable and that only delegates listed on the official Church Delegate Form submitted by their church will be issued a Delegate Pass.

Payment is required in advance or immediately upon arrival at check-in. Final note: If your name is not listed on the Official Church Delegate Form, you will not be seated as a delegate.

Pay By Check Instructions

Church and personal checks are authorized for acceptance by mail or at check-in the day of conference. Mail in checks must be received no later than May 15th. If a check has not been received by that time and is not recorded at the time of arrival a credit card payment or another check must be presented to receive delegate badges and meal wristbands. Make all checks payable to the CMCHQ and annotate with General Conference Delegate Fees. Mail to PO Box 9 Florence, MS 39073.
